Job Description Webster Family Medicine – Located in Webster, New York – is looking for a new LPN to join their practice! Supervised by a Registered Nurse or Physician, the LPN guides our patients through a seamless process to ensure that their care is of the highest quality and delivered in the most efficient manner. Directing the patients through the appointment, provision of nursing services during care, and phone triage. Collaborates in care‑planning discussions, based on RN, MD or other providers – determines urgency of care needs, triage of emergent care situations to the appropriate provider, and scheduling of acute care appointments to maximize patient care opportunities and unit efficiency. The LPN maintains the flow of the provider schedule during patient care sessions minimizing patient wait time whenever possible. Occasional evening and weekend schedules may be required.
Requirements
Licensed Practical Nurse in NY State
Minimum one year of LPN experience
Outpatient primary care and/or pediatric experience preferred
Current American Heart Association CPR and AED certification
Must successfully pass medication administration proficiency exam
Full‑time employment, 36 hours per week, day shift
