Position Summary: The Family Care Coordinator (FCC) provides intensive family support services to families of infants and young children who have or are at risk of developing disabilities or developmental delays. The FCC provides each family with a comprehensive orientation to early intervention and the Family Support Planning process. This is a mix of office work and field work. Reliable transportation and a clean driving record will be needed.

Responsibilities

Establish rapport and maintain a continuous relationship with each family during the child’s first 3 years of life, providing support and advocacy as the family works on their Individualized Family Support Plan (IFSP).

Regularly meet with families in person, by phone or via telehealth platforms to orient them to the Early Steps program, provide screening assessments, schedule service provision, update the child’s IFSP and provide information and referrals.

Based upon the wishes of the family and in collaboration with them, other Developmental Center team members and involved agencies, work to meet family needs—such as the need for therapies, special equipment, counseling, information on parenting or prematurity, or assistance in locating other community resources.

Act as a liaison between the family and multiple agencies, and help families make informed choices about services for themselves and their children with special needs.

Ensure all activities are appropriately documented in case files, required data is gathered, and billing procedures are completed to support Targeted Case Management as required by the Florida Department of Health Early Steps State Office (ESSO) and Medicaid Program Guidelines. Maintain confidentiality of data and information in compliance with HIPAA regulations and Orlando Health policies.

Perform intake coordination services: receive and process all incoming referral phone calls, faxes and emails; describe program services and eligibility requirements to caregivers and referral sources; gather all pertinent information from caregivers to complete intake paperwork; research child’s information in the program database and Medicaid system to verify status and coverage.

Create and maintain child’s electronic and paper records within 24 hours of intake.

Schedule and participate in multidisciplinary team evaluations of children to determine eligibility for the Early Steps program; schedule and participate in transition meetings with local schools when children turn three years old; serve as the primary point of contact for caregivers of children for the duration of their time in the Early Steps program, which may be up to 36 months.

Review each child’s IFSP every six months to assess progress and provide feedback to caregivers; schedule and participate in annual IFSP updates for each child on the caseload.

Support and encourage parents/caregivers to become active participants in their child’s IFSP and development.

Maintain reasonably regular, punctual attendance consistent with Orlando Health policies, the ADAAA, FMLA and other federal/state and local standards; maintain compliance with all Orlando Health policies and procedures.

Qualifications Education/Training

Bachelor’s degree from an accredited college or university with major emphasis in early childhood, social work, special education, child development, health education, psychology, and/or interdisciplinary sociology.

Bachelor’s degree in another area may be considered for this position if the candidate has additional experience (see below).

Must complete a service coordination apprenticeship and enroll as a service coordinator with the Children Medical Services Early Steps State Office and with Medicaid Targeted Case Management within 90 days of hire.

Licensure/Certification

Valid Florida driver’s license with no more than six (6) points, at least 21 years of age, and must be insurable under the Orlando Health insurance carrier’s requirements and standards.

Experience

One (1) year of experience working with young children and early intervention is required.

If the bachelor’s degree is in another area, the candidate must have three (3) years of experience working with young children and early intervention.
