Overview To provide hospital patients with psychosocial and continued care planning intervention, serve as a community liaison, and provide education in compliance with applicable hospital, departmental, and governmental regulations and policies. High-level communication skills, excellent oral and written communication, and interpersonal skills are essential.
Responsibilities
Manages referrals related to patients with psychosocial needs and determines appropriate interventions and strategies to meet those needs.
Assess patient needs and determine mode of intervention, using appropriate age and specific knowledge about group dynamics.
Document social work intervention and ensure all inappropriate referrals are channeled correctly.
Coordinate assessments and develop care plans in accordance with accepted social work policy.
Implement plan of intervention preparatory to discharge or initiating continued care plan in compliance with departmental and governmental regulations.
Act as intermediary with health care insurance providers, advocating for patient/family to obtain approval for coverage.
Execute plan of discharge/continued care and notify involved parties within 24 hours of receipt of discharge authorization.
Arrange transportation for patient where needed in accordance with hospital procedure.
Act as liaison with the community and serve as a referral source.
Coach, mentor, and train new staff in assigned duties to achieve a standard of excellence in the department.
Required Qualifications
Bachelor’s degree in Social Work
Preferred Qualifications
One year of experience preferred, preferably in a health care setting.
For ElderONE Employment: One year experience with frail elderly population preferred.
Education MS: Social Work (Required)
Licenses / Certifications LMSW – Licensed Master Social Worker – New York State Education Department (NYSED)
Physical Requirements Sedentary work: Exerting up to 10 pounds of force occasionally. The job involves sitting most of the time, with brief periods of walking or standing.
Pay Range $24.75 – $34.00 per day.
