What You’ll Do
Provide skilled nursing care in patients’ homes per physician orders.
Complete SOC, ROC, recertifications, and routine visits.
Accurately document clinical findings, including OASIS.
Communicate changes in patient condition to physicians and care team.
Collaborate with PT, OT, ST, HHA, and Medical Social Services.
Support timely admissions and 24/7 continuity of care.
Maintain compliance with Medicare and HIPAA requirements.
What We’re Looking For
1+ year RN experience (home‑health preferred).
OASIS experience strongly preferred.
Strong assessment, documentation, and time‑management skills.
Ability to work independently in the field.
