Job Purpose:
Physical therapists contracted or employed through the organization are responsible for providing physical therapy services and for adherence to all conditions outlined in the personnel service agreement.
Responsibilities
Provides physical therapy services to patients in accordance with the physician’s written plan of care. Services include assessment, treatment planning, and implementation of therapeutic interventions.
Assesses and evaluates patients’ functional status, including muscle strength, mobility, gait, range of motion, rehabilitation potential, and home environment, and determines physical therapy priorities.
Develops, initiates, and directs individualized physical therapy treatment programs to meet patient goals.
Instructs and educates patients, families, caregivers, and involved personnel on therapeutic exercises, treatment goals, safe techniques, and the use and care of therapeutic and prosthetic devices.
Monitors patient response to treatment and reports changes in condition or progress to the appropriate personnel.
Identifies patient and family/caregiver needs for additional home health services and makes referrals as appropriate.
Prepares and submits required clinical documentation, including evaluations, progress notes, and clinical summaries based on patient outcomes and attainment of goals, in accordance with organizational policy.
Participates in interdisciplinary case conferences and discharge planning.
Provides consultation and education to families, caregivers, and nursing personnel as indicated.
Participates in peer consultation and quality improvement activities.
Supervises physical therapy assistants and, when applicable, home health aides in accordance with organizational policy and state regulations, including required supervisory visits and evaluations.
Performs other job‑related duties as assigned.
