Qualifications and Skills
Registered Nurse with a current license in the state of practice or compact licensure
Two years hospice nursing experience
Excellent communication skills
Excellent interpersonal skills
Professional demeanor and appearance
Current CPR certification
Responsibilities / Essential Functions
Assess, plan, implement and evaluate the patient plan of care
Perform initial and comprehensive assessments
Develop a plan of care centered on issues, goals and interventions based on patients’ goals for care
Initiate patients’ plan of care under the direction and orders of the physician
Monitor and evaluate the plan of care in coordination with the interdisciplinary team
Effectively manage patient symptoms in coordination with the physician
Educate patients, families and caregivers on techniques for in‑home care and end‑of‑life care
Present patient clinical status and progression of decline in IDT meetings
Timely notify physician, nurse supervisor and other interdisciplinary team members of patients’ status, changes in condition and other relevant information
Supervise the licensed practical nurse and effectively delegate assignments
Train and supervise hospice aide care
Document at point of care; documentation must be accurate and reflect patient condition and decline
Complete education, participate in in‑services, continuing education programs and seminars
Provide education to hospice employees and community partners
Maintain compliance with Medicare Conditions of Participation, state and federal laws, Arizona Nurse Practice Act, Joint Commission accreditation standards and hospice policy and procedures
Actively participate as a member of the team and demonstrate the organization’s mission and values in performance and behavior
