Full-time position working 40 hours per week
Easterseals Florida is hiring a compassionate and organized
Case Manager
to support families navigating services through our Autism Center. This role is perfect for someone who thrives in a family-centered, collaborative environment and is passionate about helping children and caregivers access the resources they need.
What You'll Do
Welcome families, build rapport, and serve as their primary point of contact throughout the assessment and service process
Guide families through autism screenings and assessments, providing education and support based on results
Act as client advocate regarding intervention and treatment options
Manage a client caseload, identify goals and develop individualized service plans
Coordinate referrals to Easterseals programs and community resources
Collaborate closely with physicians, therapists, internal teams, and community partners to ensure quality, timely, person-centered care
Maintain accurate documentation in the electronic medical record and assist with assigned data tracking and reporting
Stay informed on current research, services, and community resources to provide up-to-date information

What You Bring
Bachelor’s degree in Social Work, Psychology, or a related human services field (required)
At least 1 year of social work or human services experience (required)
Knowledge of Autism Spectrum Disorder and family support needs (strongly preferred)
Ability to collaborate with multidisciplinary teams and communicate effectively with families
Strong organizational skills, initiative, and flexibility
Proficiency in EMR systems and Microsoft Office
Professionalism, integrity, tact, and good judgment
Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)
