Job Summary The Quality Coordinator is responsible for assessing and meeting the needs of patients and families. The Quality Coordinator supports the Care Management team, ensuring all pertinent patient information is shared amongst team members. They regularly assess the practice for compliance within various programs and offer suggestions to improve workflows. The Quality Coordinator makes certain the patients gaps in care are being appropriately addressed.
