The Nurse Supervisor, OTRC Behavioral Health, collaborates closely with the Directors of High Intensity Mental Health, Director of Behavioral Health Outpatient Services and the Director of Nursing. This position will report directly to the Director of Nursing.

Behavioral Health services include outpatient mental health care for severe and persistent mental illness (SPMI), Assertive Community Treatment (ACT), and Intensive Case Management (ICM) services out of the Old Town Recovery Center location, and the Overdose Prevention Program.

Old Town Recovery Center (33 NW Broadway, Portland, OR 97209)

Schedule:

Monday - Friday, 8:00am - 5:00pm

Compensation:

Minimum annual rate of $96,000.

The final salary offered will be influenced by professional experience and internal equity.

Minimum Qualifications

Current Oregon Registered Nurse License required.

Two years’ experience as a registered nurse.

Must possess a current driver's license and access to a vehicle. Must meet CCC Fleet Safety requirements, pass an initial driver’s training within 60 days of approval, and maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.

Bachelor’s Degree in nursing preferred.

QMHP-R, at minimum, preferred.

Must meet CCC privileging requirements as required by FTCA.

Progressively increasing responsibility in nursing preferred.

Must have current BLS certification prior to start date.

Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.

Ability to adhere to Central City Concern’s drug‑free workplace and comply with the Drug‑Free Workplace Act of 1988.

Must pass a pre‑employment drug screen, TB test, and background check, including clearance by the DHS Background Check Unit.

Must adhere to agency’s non‑discrimination policies.

Ability to effectively interact with co‑workers and clients with diverse ethnic, religious, cultural and sexual backgrounds, treating all with respect and dignity.

Will be required to carry an agency cell phone for work use; the phone will be provided by Central City Concern.

Essential Duties and Responsibilities

Administrative and Clinical Supervision:

Regular supervision of nursing staff per regulatory standard, including 90‑day, ongoing and annual performance evaluations; plan and facilitate monthly RN department meetings; provide at least 2 hours of supervision per month for each full‑time staff supervised, including one hour of face‑to‑face contact for each person supervised or a proportional level of supervision for part‑time staff. Supervision may take place in the community during clinical interactions.

Nurse Operations:

Responsible for staffing adequate nurse coverage for BH teams, coordinating nurse workflows, evaluating nursing productivity, and initiating and engaging in process improvement.

Nursing Practice:

Serve as a clinical consult for triage and/or clinical visits at least 50% of the work week, including:

Participate in evaluating clients for risk of harm and initiate Director’s Custody transport holds when appropriate.

Triage and ongoing assessment of clients’ physical and behavioral health needs in clinic and community settings, providing appropriate referrals, coordinating with providers for treatment and follow‑up, and addressing health concerns while supporting client autonomy.

Administer treatments and medications.

Participate in outreach activities as necessary to offer services and engage in a non‑coercive manner; escort clients to meetings and appointments as necessary.

Participate in the rotation of carrying after‑hours pager and providing holiday coverage with other team members.

Provide clinical care in any location certified to provide behavioral health and primary care at CCC.

Document all services rendered, including client interactions and service plans via CCC EHR.

Staff Development:

Coordinate onboarding and ongoing training as necessary.

Policy Improvement:

In partnership with operational leaders, assist in the development and implementation of new protocols as needed to ensure compliance with regulatory bodies.

Incident Response:

Partner with BH staff to respond to behavioral and urgent incidents.

Coordination:

With team managers to ensure nursing practice aligns with workflows, staffing and regulatory requirements.

Customer Service:

Respond to clients, internal and external service providers with respect and efficiency.

Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.

Attend all mandatory CCC trainings in a timely manner.

Perform other duties as assigned.

Skills and Abilities

Experience caring for individuals with complex substance use and advanced behavioral health needs using trauma‑informed principles.

Ability to coordinate and relate effectively and respectfully with all members of the team.

Ability to multi‑task and remain calm in a busy and stressful environment.

Demonstrated leadership skills, performance coaching, and clinical guidance.

Ability to work independently, prioritize workload, problem‑solve, and complete tasks in a timely fashion.

Advanced skills in counseling and assessment with individuals and groups, including assessing danger to self or others.

Ability to maintain composure and work therapeutically with clients who may be verbally or behaviorally escalated and/or intoxicated.
