LTSS Service Coordinator (Case Manager)

Hiring in Southwestern Wisconsin counties:
Pepin, Buffalo, Trempealeau, Jackson, Clark, La Crosse, Monroe, Juneau, Vernon, Sauk, Richland, Crawford, Grant, Iowa, Lafayette, Green.
Location:
This field‑based role primarily operates in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training.
How You Will Make an Impact
Perform face‑to‑face program assessments to identify and coordinate individuals’ waiver (such as Family Care Partnership) and behavioral or physical health needs.
Use tools and predefined identification processes to identify members with potential clinical health care needs, serving as the single point of contact with the clinical health care management and interdisciplinary team.
Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities to ensure cost‑effective and efficient utilization of long‑term services and supports.
Document short‑ and long‑term service and support goals in collaboration with the member’s chosen care team, including caregivers, family, natural supports, service providers, and physicians.
Act as mentor, subject‑matter expert, or preceptor for new staff, aiding their formal training and participating in process improvement initiatives.
Submit utilization/authorization requests to utilization management with documentation supporting the individual’s care plan.
Report critical incidents to appropriate internal and external parties such as state and county agencies (Adult Protective Services, Law Enforcement).
Assist in and participate in appeals, fair hearings, member grievances, appeals, and state audits.
Minimum Requirements
BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent combination of education and experience.
Preferred Skills, Capabilities and Experiences
BA/BS degree in a health care related field.
Assisted living or social work experience.
Waiver experience.
Comfortable using technology.
Travel to work sites and other locations as necessary.
