Client Care Coordinator Job Summary The Client Care Coordinator will work at a shelter or temporary lodging site, managing a housing caseload and coordinating care to help individuals and families end homelessness. The role coordinates care with existing patient‑centered medical homes (PCMH), primary care providers (PCP), and community agencies, ensuring services such as crisis, urgent care, residential treatment, medication‑assisted treatment, and outpatient behavioral health are delivered.
Responsibilities
Coordinate and monitor housing and community‑based services for clients.
Use housing search software to match clients to suitable units.
Maintain detailed data entry in the established system and prepare end‑of‑shift reports.
Document and track interactions according to AAC, ADHS/DBHS, CARF, and CBI policies.
Collaborate with multidisciplinary team members including nurses, counselors, and clinical leads.
Skills / Requirements
High school diploma or GED required; associate degree in a field related to behavioral health preferred.
Minimum six months recovery from substance use or mental health disorders.
Minimum six months paid related work experience with exposure to behavioral and medical crisis situations.
1‑3 years full‑time health care related work experience preferred.
Valid identification.
Will complete CBI Peer Certification within 90 days of hiring.
Arizona Fingerprint Clearance Card (Program Specific).
