Job Description Altercare Bucyrus is seeking an LPN Alzheimer’s Program Coordinator for their specialized, secure unit. Deliver programming with and through your CNAs to enrich the lives of the residents. Touch the resident's lives clinically, emotionally, spiritually. Job Responsibilities
Oversee and supervise all aspects of care and programming for dementia residents.
Design and implement creative activities for dementia residents.
Develop care plans and perform assessments.
Conduct resident screenings and family interviews.
Job Requirements
Licensed Practical Nurse (LPN) with valid Ohio License.
Experience in development and facilitation of activity programs preferred.
Prior experience in a skilled nursing, long‑term care, or geriatric setting preferred.
Knowledge of Alzheimer’s disease and related dementia appreciated.
