Overview Job Description & Requirements
Responsibilities
Assess referred patients that require rehabilitation services, provide direct patient care, and determine medical condition, functional capabilities, limitations, and potential for rehabilitation.
Establish and administer a treatment program with specific goals based on patient capacity and tolerance under physician direction.
Set realistic and achievable goals for patients, document and record patient condition, and educate patients and families in an appropriate physical therapy method.
Collaborate with the care team to plan and evaluate goals for each patient.
Qualifications
Must have graduated from an accredited school.
Current state license as a Registered Physical Therapist / PT or be in the process of licensure.
Current CPR certification.
Develop a plan of care for each physical therapy patient.
Provide skilled physical therapy services / interventions in accordance with physician orders.
Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.
Collaborate with all disciplines to plan and evaluate team goals for each patient.
