Licensed Practical Nurse, Telemetry Care Unit (Full Time Nights) Job Description

Provides basic therapeutic nursing care according to established standards of practice under the direction and supervision of the RN. Care is provided to patients and caregivers whose requirements are routine and standardized. The LPN assists the RN and healthcare team to achieve desired patient outcomes, relying on education, training and established guidelines. Initially requires instruction and guidance, evolving to work under general supervision.

Responsibilities

Patient Care

Provides direct patient care utilizing the nursing process under the direction and supervision of the RN.

Reviews diagnostic test results and reports abnormal findings and changes in patient condition to the RN, physician and appropriate healthcare team members in a timely manner.

Administers prescribed medications, therapies and treatments in accordance with practical nursing standards.

Maintains and reviews patient medical records, notes and physician orders, documenting all patient care activities in accordance with documentation standards.

Implements components of the plan of care within the LPN scope of practice and competency to promote desired outcomes.

Assists with discharge planning and carrying out the patient teaching plan under the direction of the RN.

Evaluates and maintains the environment to promote patient and healthcare team safety.

Provides compassionate basic nursing care, maintains patient privacy and confidentiality.

Communication and Collaboration

Collaborates with the supervising RN on the execution of the patient’s plan of care.

Assists physicians, RNs and health care team members with data collection, assessment, planning, intervention and evaluation of patient and family care to achieve desired outcomes.

Advocates for the patient and supports the patient’s psychological, ethical, spiritual and cultural needs by collaborating with the RN and healthcare team.

Seeks out healthcare team members to serve as a resource to increase practice knowledge.

Keeps informed of unit initiatives, supports the outcomes of the team/committees’ work into practice.

Participates on unit teams, work groups and/or committees, anticipating needs and voluntarily assisting others.

Quality Improvement

Demonstrates knowledge of unit-based quality improvement initiatives.

Participates in quality improvement initiatives using standards, guidelines and pathways for care delivery.

Professional Development

Identifies own learning needs based on self‑evaluation and progress from the previous year’s goals.

Assists with the orientation of new employees under the supervision of the RN.

Seeks opportunities to further increase practice knowledge.

Accountable for all documentation and completion of all required continuing education and competencies relevant to area of practice as defined by clinical management.

Qualifications

Graduate of an accredited LPN school

Proficient in English, verbal and written communication skills and computer skills

Licensed as LPN in the state of Louisiana or have a work permit and BLS certification
