Job Description
Performs behavioral health intakes, psycho‑social assessments, care plans, and psychotherapy with patients and families coping with behavioral health disorders.
Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community.
Collaborates with the multi‑disciplinary team in establishing and documenting psycho‑social determinants of health to attain safe discharges and avoid emergency hospital admissions.
Serves as a consultant and educator to the multi‑disciplinary team regarding psycho‑social components of illness and hospitalization.
Maintains professional competency by participating in continuing education and appropriate learning experiences, both internally and externally.
Qualification Requirements Master’s (Required) Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Cardiopulmonary Resuscitation (CPR) - Accredited Issuing Body, Clinical Social Worker License (LCSW) - EV Accredited Issuing Body, Licensed Mental Health Counselor (LMHC) - EV Accredited Issuing Body
Pay Range $26.08 – $48.50
