Provide comprehensive primary care exclusively for seniors (65+ Medicare/Medicare Advantage)
Longer, unrushed visits to foster meaningful patient relationships
Collaborate closely with experienced physicians and multidisciplinary care teams
Apply evidence-based protocols to support informed, patient-centered decisions
Thrive in a culture that values respect, teamwork, and clinical excellence
Practice Environment: Senior-focused outpatient setting designed for quality care
Small patient panels allowing for deeper patient engagement
Value-based care model with care coordination, analytics, and robust administrative support
Streamlined EMR workflows to reduce documentation burden
Modern clinic with emphasis on preventive care, wellness, and patient education
Leadership committed to APP growth, mentorship, and recognition
Compensation and Benefits
Lucrative base salary with bonus potential of near $25K+ and a sign-on bonus as well

Candidate Requirements
Licensed APRN in Florida and graduate of an accredited nursing program
1-3 years of primary senior care experience preferred
Confident managing chronic conditions such as diabetes, COPD, CHF, and hypertension
Proficient in EMR documentation with strong organizational skills
A natural communicator with genuine compassion and strong follow-through
Passionate about senior-focused, value-based care and patient advocacy
