Licensed Vocational Nurse (LVN)
to join our Houston team, providing high‑touch clinical support to hospice patients in the comfort of their own homes.
In this role, you aren’t just "checking vitals"—you are a vital part of a compassionate care team. Working under the guidance of a Registered Nurse (RN), you will provide the clinical expertise and emotional support that allow patients and their families to focus on what truly matters: quality time together.
What You’ll Do: Specialized Clinical Care
Clinical Monitoring:
Diligently monitor patient conditions and vital signs, identifying and reporting subtle changes in status to the RN to ensure proactive care.
Treatment & Medication:
Administer medications and perform treatments as prescribed, ensuring pain management and symptom control are prioritized.
Comfort & Dignity:
Provide hands‑on nursing care focused on the comfort, hygiene, and dignity of hospice patients.
Patient & Family Advocacy:
Collaborate closely with a multidisciplinary team of healthcare professionals to address the physical and emotional needs of both the patient and their loved ones.
Documentation Excellence:
Maintain accurate, timely, and detailed documentation of care provided, ensuring compliance with hospice care plans and state regulations.
Who You Are
Licensed Professional:
You hold a current
LVN license
in the state of Texas.
Experienced Caregiver:
You have at least
one year of nursing experience , with a strong preference for those who have worked in hospice or home care environments.
Clinically Sharp:
You possess strong clinical skills and the ability to strictly follow complex care plans while maintaining a high attention to detail.
Empathetic Communicator:
You have excellent interpersonal skills and the emotional intelligence required to support families during sensitive times.
Self‑Reliable & Collaborative:
You are comfortable working independently in a home health setting while remaining a seamless collaborator with the broader care team.
