Job Overview We’re offering even more great benefits when you join our team! We are looking for a nurse practitioner who is committed to providing compassionate, high-quality care to patients and families in palliative settings.
As a hospice and palliative care nurse practitioner, you will provide diagnosis, treatment, and care under physician supervision, focusing on palliative care and supporting families along their healthcare journeys.
Responsibilities
Provide diagnosis, treatment, and care under physician supervision, focusing on palliative care.
Assist families along their health care journeys with compassion and support.
Participate in interdisciplinary team meetings and care conferences.
Use professional development opportunities to advance practice.
Qualifications
A heart to serve patients and families with passion.
Education:
Graduate of an accredited master’s, post‑master’s, or doctoral nursing program with formal NP preparation.
Licensure:
Current unencumbered APRN licensure as a Nurse Practitioner, with full DEA privileges and national certification in the NP specialty.
Experience:
At least 2 years within chronic disease management/long‑term care in the last 5 years. Hospice and/or palliative care background preferred.
Required:
Reliable transportation; able to sit, stand, bend, move intermittently and lift 80‑100 lbs.
