Job Overview The Licensed Social Worker (LSW) provides psychosocial assessment, counseling, care coordination, and resource navigation for patients undergoing cancer treatment and their families.
This role supports patients in managing the emotional, social, financial, and practical challenges associated with a cancer diagnosis while collaborating closely with the interdisciplinary oncology care team.
Qualifications Required:
Master's degree in Social Work
Preferred:
One year previous experience in a healthcare setting
Bilingual in English and Spanish
Certifications and Licenses Required

Scheduling Requirements
Shift working, Monday-Friday 8am-4pm
Full-Time (FT)
Essential Functions
Conduct comprehensive psychosocial assessments for oncology patients and caregivers
Provide counseling and emotional support related to diagnosis, treatment, survivorship and end of life issues
Assist patients with coping strategies for anxiety, depression, grief and treatment‑related stress
Identify and address barriers to care, including transportation, housing, employment, insurance and financial concerns
Performs clinical assessments, interventions, and counseling with patients, families, and groups to achieve optimal psychosocial functioning
Coordinates and facilitates referrals to community agencies to achieve an optimal interface between patient/family needs and community resources
Participates in multidisciplinary case conferences, staff education and support, and professional education to enhance knowledge and teamwork for optimal patient care
Involvement in community networking to increase knowledge and availability of services to patients and families
Administrative activities include staff meetings, statistical reporting, quality assurance and other assigned duties to provide accountability and support objectives of the cancer center
Assesses and responds to identified needs of patients and families, coordinating with medical and nursing staff, community programs, and other members of the psychosocial team to provide optimal patient care
Coordination of complex systems to provide optimal patient care and enhance the effective functioning of these systems in delivery of services
Complies with regulatory requirements appropriate for the position i.e. Joint Commission
