Job Purpose
We are seeking a Nurse Practitioner/Physician Assistant to work in collaboration and consultation with physicians, staff RNs, and other health care professionals to provide high‑touch, compassionate care to patients and families while coordinating and managing care activities. The NP/PA will practice within a collaborative agreement with a physician(s) to diagnose, manage, and prevent acute and chronic illnesses and promote wellness, exercising independent nursing functions based on standards of care.
Key Responsibilities
Perform comprehensive adult/geriatric admission history and physical exams.
Identify, diagnose, and document episodic, acute and chronic disease in adults.
Recognize conditions that place seniors at risk.
Pursue feedback, continuing education, and professional development.
Effectively reconcile medications.
Document needs of clinically complex patients.
Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary personnel.
Participate in Quality Assurance and Performance Improvement (QAPI) activities.
Assess medical necessity of ancillary services and communicate processes.
Coordinate care transitions across levels of care, ensuring ICP is communicated no later than 24 hours.
Minimum Education Required Graduate of an accredited Adult or Family Nurse Practitioner Master’s Degree program or Physician Assistant Studies.
Minimum Experience Required Minimum 2 years clinical experience as a Nurse Practitioner or Physician Assistant in a geriatric, adult, or family practice setting, including comprehensive adult/geriatric exams.
Minimum Licensure / Certification Required By Law
Current and unrestricted RN and NP licensure, or PA licensure in state of practice. Board certification by AANP or ANCC in Adult, Family Practice and Geriatrics or Adult‑Gerontology Primary Care. Current CPR certification.
Active DEA license.
Active national provider number (NPI) and Medicare provider number.
Eligible to participate in Medicare program as a provider and registered.
Additional Qualifications (Preferred) Graduate of an accredited Geriatric, Adult or Family Nurse Practitioner Master’s Degree program. Previous long‑term care experience strongly preferred.
Knowledge, Skills, Abilities
Relevant clinical work experience with the population served.
Review and interpret medical and social histories.
Interpret laboratory data, recommend follow‑up, reconcile medications.
Manage both medical and behavioral chronic and acute conditions in collaboration with a physician or specialty provider.
Synthesize information, diagnose, and create comprehensive plans of care.
Complete all regulatory required monthly, quarterly, and yearly visits and acute visits as needed.
Maintain appropriate, compliant documentation within EMR.
Accurately code diagnoses with ICD‑10.
Review orders and interventions for appropriateness and response to treatment.
Participate in family conferences; provide advanced care planning and education.
Address pharmacy recommendations and pharmacist communications regarding residents.
Provide dedicated on‑site coverage during work hours.
Practice ethically according to scope and standards of practice and board certification.
Meet rotating on‑call responsibilities and after‑hour coverage.
Travel between care sites as required.
Perform other duties as assigned.
