Acute Care Coordination is a collaborative process of assessment, planning, facilitations, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality and cost effective outcomes. $23.49-$38.86/hr. based on previous work experience. Education
Requires a MSW or Bachelor's degree (BSW) in Social Work with 2 years of experience will be accepted in lieu of the MSW. Experience
Prefer 1 year of experience if MSW or 2 years of experience required if BSW. Medical Social Work experience preferred. (Above requirements can be met by the equivalent combination of education and experience). Licensure / Certification
Either degree must be from a university accredited by Council of Social Work Education. Licensure by Oklahoma State Board of Licensed Social Workers preferred. Basic Life Support (BLS) training or re-training is required during orientation prior to providing patient care. Current Basic Life Support (BLS) is required to be maintained thru re-training for duration of employment. Schedule
