Client Services Coordinator Silverchain has an exciting opportunity for a

Client Service Co-ordinator to join our teams within the Albany

service centreon a fixed Term, part time basis. The working hours are 8am - 4pm, 3 days per week. You will travel to our clients in Albany and surrounding areas.

The Client Service Co-ordinator willfacilitate the effective and efficient entry and onboarding of new clients to Silverchain. Ensuring that all assessed care needs and approved Support at Home funding is discussed with clients, to highlight the comprehensive service delivery from Silverchain, which can support clients to achieve their goals and deliver the best outcomes for the client, their carers’ and families.

Travel allowance of $1.03 per k/m

17.5% annual leave loading and excess leave cash out options

What we are looking for

Tertiary qualification or Certificate IV or Diploma in Aged Care Work; Disability Work or Community Services (Service Co-ordination) to ensure service delivery and outcomes.

Demonstrated experience in onboarding new clients and creating/developing/designing effective and sustainable care packages.

A demonstrated understanding of aged, community, disability and health care and the various applicable programs (e.g. Veterans’ Home Care; Home and Community Care; Disability Services and Care Packages) to support service delivery within a competitive and growing industry.

Demonstrated ability to negotiate with and influence clients toward pathways that deliver quality client outcomes and experience and supports the client’s independence and capability.

Demonstrated flexible approach to care delivery to facilitate positive and safe outcomes for client careers’ and Silverchain staff.

A commitment to excellence in customer service to ensure the consistent provision of exceptional service to internal and external stakeholders.

Well-developed communication (written, verbal and listening) and interpersonal skills to develop effective relationships and facilitate employee communications.

Demonstrated ability to work autonomously and as part of a team always with a team focus to participate and contribute to the success of service centre and organisational objectives and vision.

Demonstrated time management and organisational skills to effectively plan and prioritise workflow.

Demonstrated computer literacy in word processing, spreadsheets, and presentation software to prepare effective correspondence and documentation.
