Base Pay Range
$21.00/hr - $23.00/hr
This position processes health information under the direction of the HIM Director or designated supervisor. The role includes coordinating physician medical record completion and performing the quantitative analysis of all medical record patient types based on standards established by Title 22, CIHQ, Conditions of Participation, and the Medical Staff Rules and Regulations.
Skills and Requirements
Physician chart completion and chart deficiency analysis
Basic keyboarding skills (typing speed of 35 wpm)
Knowledge of medical terminology and familiarity with computers
Ability to categorize forms/documents within the medical record
Detail‑oriented and self‑motivated
Ability to work under continual deadlines and variable requirements
Ability to focus and maintain accuracy despite interruptions
Courteous, tactful, and cooperative
Proficiency with standard office equipment (computers, photocopier, fax, scanner)
Knowledge of Title 22, CIHQ, Conditions of Participation, Medical Staff Bylaws and Rules and Regulations
Education / Experience
Knowledgeable of Windows software
3–5 years of medical record experience in an acute care setting
Previous experience with electronic health record applications
Preferred
High School graduate or equivalent
Knowledge of physician record completion and HIPAA
Proficiency in medical terminology
Duties and Responsibilities
Safeguard and preserve patient’s protected health information in compliance with HIPAA, state, and federal regulations.
Ensure a safe patient environment and adherence to safety practices.
Resolve biophysical, psychological, educational, and environmental needs of patients or significant others when administering care.
Notify physicians of medical records requiring completion in accordance with Medical Staff Bylaws and regulatory requirements; maintain documentation of notifications.
Enforce medical staff guidelines pertaining to record completion; perform daily counts of pending records and monitor unsigned/undistributed orders.
Retrieve incomplete records and assist physicians directly in completing their records electronically.
Activate temporary suspension of medical staff privileges when records are not completed promptly; communicate suspension information to relevant departments and maintain documentation for reporting.
Analyze and re‑analyze incomplete patient records to ensure completeness; update chart tracking systems.
Scan loose filing into ChartMaxx System.
Use ChartMaxx for deficiency analysis and reporting.
Adhere to daily productivity standards.
Oversee all incomplete record activities and functions.
Assist physicians with record completion issues and escalations.
Complete RLDatix Incident Reports for potential compensable events.
Conduct record review functions and provide data to the Director or supervisor.
Perform basic eScription1 monitoring.
Operate office equipment (photocopy, fax, computers, scanners, telephones).
Communicate effectively with all customers served.
Abide by compliance requirements, enforce policies, and perform duties ethically.
