Job Description Description: a. Ability to answer Crisis hotline b. Availability to attend monthly team meeting outside of shift if required · Maintain assigned caseload of weekly billable insurance hours. · Responsible for ensuring services are provided in the community. · Participating in the person-centered planning process. · Providing direct community-based behavioral health services and support for the individual and family as indicated in the Person-Centered Plan. · Monitor and track consumer hours and annual intake dates and complete assigned. · Assisting the Case Manager in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward goals. · Responsible for completing the OnTarget tasks assigned within 24 hours. Assisting the Case Manager with consulting identified non-medical providers, engaging in community and natural supports and including their input in the person-center/ed planning process. HOURS: i. Tues 10:00a - 6:30p ii. Wed – Sat : 6:00p-2:30a Requirements: · One-year documented experience in the delivery of mental health treatment to children and /or adolescents. · High School diploma (additional 5 years’ mental health experience preferred) · Human service field with less than one year of full-time, post-graduate degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional with less than one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision shall be provided by a qualified professional with the population served until the individual meets one year of experience. Must be highly organized and detail-oriented with ability to be self-directed and motivated, and to securely manage confidential information.
