Home Health Aide Position Description
Place of Employment: Client homes
Home Health Aide Job Purpose: Enables clients to stay in their homes by monitoring and recording their condition; providing support and personal services; and teaching the client and their families.
Summary: This is a front-line position acting as a representative of Aluma Home Care and providing home healthcare services with RN supervision. You will mostly work independently of co-workers or supervisors and therefore must balance good judgment and initiative along with the policies of the company.
Monitors client condition by observing physical and mental status.
Supports clients by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks; running errands.
Assists clients by providing personal services, such as bathing, dressing, and grooming.
Helps clients care for themselves by teaching use of cane or walker, special utensils to eat, and equipment for personal hygiene.
Helps family members care for the client by teaching appropriate ways to lift, turn, and reposition the client; advising on nutrition, cleanliness, and housekeeping.
Records client information by making entries in the client journal or care log; notifying nursing supervisor of any changes or unusual conditions.
Maintains a safe, secure, and healthy client environment by following policies and procedures; following prescribed dietary requirements and nutrition standards.
Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements.
Your duties are not limited to Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) to maintain the dignity and independence of our clients. It should be noted that emphasis is for you to involve the client in the Care Plan to help provide the client with the basic standards of healthy living.
General Notes: You will be required to maintain a safe working environment for yourself and your clients. There may be emergency situations where you will have to react with sound judgments.
Home Health Aide - Qualifications and Requirements
Valid driver’s license or State ID.
Ability to work independently
One-year experience with direct caregiving and/or a current (less than 2 years) HHA/CNA or STNA certification.
Excellent communication and listening skills
Availability; Contract is signed by caregiver upon hire. It is the caregiver’s responsibility to come into the office and sign a new contract if caregiver's availability changes.
Caregivers weekly schedule will be made by our scheduling department and communicated via phone. It is the caregiver’s responsibility to check their schedule on a regular basis.
Any changes made to the schedule will be communicated to the caregiver.
