Hours:

8 AM – 4:30 PM (8‑hour shift); weekend as needed; on‑call: no.

Hourly pay range:

$47,060 – $60,730 – $68,020.

What You Will Do:

Provide consultation, psychosocial counseling and support services to the Sharp HealthCare customer base. Mobilize patients and families’ personal and environmental resources to receive maximum benefits of medical care and achieve the fullest personal and family functioning.

Required Qualifications

Master’s Degree in Social Work.

Preferred Qualifications

Hospital experience.

California Board of Behavioral Sciences Licensed Clinical Social Worker (LCSW).

Essential Functions

Assess psychosocial needs and complete psychosocial assessments, identifying key clinical issues such as grief, loss, adjustment to illness, psychiatric concerns, developmental milestones, and life transitions that impact functioning.

Document assessment findings per department guidelines and consult with team members to develop treatment plans; maintain ongoing communication with multidisciplinary team members regarding patient status and follow‑up; document ongoing progress in the medical record; attend rounds, document consultation, recommendations, interventions, and treatment plans; arrange and participate in patient care conferences for high‑risk psychosocial cases.

Utilize and refer to internal hospital resources and community resources that assist patients and families; facilitate timely discharge and provide referral to ongoing community services/support after leaving hospital.

Consult and intervene in crisis situations; assist staff in de‑escalating volatile situations; educate staff on handling Code Green situations; coordinate critical incident debriefings; develop nursing education to increase staff knowledge.

Encourage patient and family participation in decision‑making; report mandatory protective concerns such as elder abuse, child abuse, and domestic violence as required by law; protect patient privacy; keep current with regulatory and legal requirements regarding privacy and confidentiality; identify bio‑ethical dilemmas and initiate appropriate response.

Develop and implement treatment goals addressing identified needs; practice appropriate boundary setting with clients; demonstrate sensitivity to cultural and ethnic diversity; provide crisis intervention and problem‑solving techniques; seek resources when faced with unusual or new situations; utilize appropriate treatment modalities including individual, conjoint, and group.

Evaluate patient/family response to interventions and revise care accordingly.
