Behavioral Health Deflection Case Manager

The BH Deflection Case Manager is responsible for providing outreachand intensive case management to individuals eligible for treatmentinstead of prosecution for minor drug offenses. Serving as a keycommunity liaison, this role promotes deflection services, worksclosely with law enforcement and legal partners, and collaborates withthe Peer Navigator and treatment team to support patient recoveryacross CTCLUSI’s five-county service area.
Notice : This is a full-time, limited-duration position funded through agrant. Current funding is secured through April 2026, with thepossibilityof renewal contingent upon continued funding.
PRINCIPAL ACTIVITIES & RESPONSIBILITIES:
Provides assertive and continuous outreach activities related todirect services for participants with criminal justice involvementand behavioral health conditions (mental health, substance use, orco-occurring disorders).
Establishes and Nurtures strong, trusting relationships withparticipants in a timely and effective manner.
Makes and manages referrals to essential direct services such assubstance use disorder and mental health treatment and recoverysupport services to ensure access and engagement in treatment basedon participants’ unique needs.
Attends community events and conferences to educate and raiseawareness about law enforcement diversion and outreach forindividuals with substance use and mental health disorders.
Conducts enrollment assistance for Medicaid for uninsuredindividuals when needed.
Follows up on obtaining required services for all participant needs.
Develops effective working relationships with project staff,community partners, and police departments.
Represents the program, TRHC, and CTCLUSI at community events.
Provides and/or collaborates on ongoing training related toprogram goals and subject matter, such as naloxone administrationand distribution.
Assesses the needs of participants with substance use and/ormental health disorders.
Assists in the development and revision of BHDeflection-associated policies, procedures, and as directed.
Assists in the development and revision of BH Deflection-relatedreports, memos, letters, forms, and other documents relevant toDeflection services.
Participates in quality and organizational process improvementactivities, team meetings, and committees when requested.
Documents all clinical encounters in identified Electronic HealthRecord (EHR) or case management system.
Assists Behavioral Health Director and other internal stakeholdersto review trends, barriers, and propose solutions to BH Deflection processes.
Collaborates efficiently and effectively while consistentlydemonstrating professionalism and maintaining positive, respectfulrelationships with internal teams, external partners, and Tribal members.
Other duties as directed by management.
LEVEL OF AUTHORITY & RESTRICTIONS:
This is not a supervisory position.
PHYSICAL & MENTAL DEMANDS:
Working with people in crisis who may have additionalbarriers/disabilities that can be mentally challenging.
Must be able to walk, talk, hear, use hands to handle, feel oroperate objects, tools, or controls, and reach with hands and arms.
Vision abilities required by this job include close vision and theability to adjust focus.
May be required to push, pull, lift, and/or carry up to 20 poundsas needed.
WORKING CONDITIONS & ENVIRONMENT:
Moderate noise level with frequent interruptions and distractions.
May be required to work in locations other than Three RiversHealth Center, such as police departments, jails, courts, or at anoutreach work location.
May be required to attend meetings, conferences and Tribal events.
Must be willing and able to work long hours, which may includeevenings and weekends as needed.
Must be willing and able to travel both locally and within theCTCLUSI service delivery area.

Experience and Skills: MINIMUM JOB REQUIREMENTS:
Must be 25 years of age or older per our vehicle use policy.
High School Diploma or equivalent.
1-year of verifiable experience providing care coordination, casemanagement, or direct services to similar populations within medicalor behavioral settings.
Community Health Worker (CHW) certification required; specialtycertifications such as Certified Alcohol and Drug Counselor (CADC)or Peer Support Specialist (PSS) preferred. Highly qualifiedcandidates without CHW may be considered if willing to obtain CHWwithin one year of hire.
Must have a valid driver’s license and be able to be insured todrive Tribal Government vehicles.
Experience and proficiency in the use of Microsoft products(Excel, Outlook, PowerPoint, Word, etc).
Experience working with vulnerable populations in a medical,behavioral health, or social service setting and experienceadministering the OHP and Medicare benefits preferred.
Ability to work independently and possess a strong sense of focus,attention to detail, and task-orientation.
Ability to work with culturally diverse families and communitieswith the ability to be culturally sensitive, appropriate and non-judgmental.
Maintain a calm and professional attitude even in the face ofcrisis and dramatic presentation.
Maintain HIPAA standards of privacy and confidentiality, ensuringthat work assignments and information gathered from day-to-day workis not shared with anyone and protects all patient and agency information.
Strong interpersonal communication and written skills.
Ability to communicate clearly and effectively in English,verbally, in writing or by other acceptable means.
This is a
safety sensitive
position and will besubject to pre-employment drug testing and criminal historybackground check, which will include fingerprinting.
