Neighborhood Adult Day Care Center
Job Description
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our healthcare team. The RN will be responsible for delivering high-quality nursing care to patients, coordinating with family members and physicians, and ensuring adherence to established policies, procedures, and safety standards.
Core Clinical Responsibilities
Health Assessments:
Conduct initial and ongoing health assessments of participants.
Monitor vital signs and observe for changes in condition.
Medication Management:
Administer medications as prescribed.
Educate clients and caregivers on medication use.
Maintain accurate medication records.
Wound Care & Chronic Disease Management:
Provide basic wound care, catheter care, etc.
Monitor and manage conditions like diabetes, hypertension, dementia, etc.
Emergency Response:
Respond to medical emergencies.
Coordinate with EMS or hospitals as needed.
Coordination & Care Planning
Develop Care Plans:
Collaborate with other staff (social workers, physical therapists, etc.) to create individualized care plans.
Communication:
Serve as liaison between participants, families, and healthcare providers.
Provide updates and education to family members as needed.
Documentation:
Maintain up-to-date nursing notes and health records.
Document changes in condition, interventions, and outcomes.
Supportive & Educational Duties
Health Education:
Teach participants about healthy lifestyle habits, disease prevention, and self-care.
Provide caregiver training.
Promote Safety & Hygiene:
Oversee infection control practices.
Ensure a safe and clean environment for participants.
