Department Case Management/Social Worker
What You'll Do
Conduct comprehensive psychosocial assessments for patients and families.
Develop and implement individualized discharge plans to ensure safe and appropriate transitions of care.
Coordinate patient care across interdisciplinary teams (physicians, nurses, therapists, etc.).
Facilitate communication between patients, families, and healthcare providers.
Identify and address barriers to discharge, including financial, social, and emotional issues.
Connect patients and families with community resources, home health, rehabilitation, or long-term care facilities.
Advocate for patients’ rights and access to necessary services and supports.
Provide crisis intervention and emotional support for patients and families.
Document assessments, plans, and progress in the electronic medical record (EMR) in compliance with hospital and regulatory standards.
What We're Looking For
Master’s degree, Social Work required.
Experience with:
Behavioral Health, homelessness, substance abuse, elderly care, Medicaid, assisted living, in facility placement is preferred.
Must be familiar with working with physicians and nurse managers.
