Responsibilities
Conduct assessments, diagnoses, and develop treatment and self‑management plans.
Provide crisis intervention and referrals as needed.
Collaborate closely with clinic staff and external partners.
Maintain comprehensive documentation in the electronic health record (EHR).
Qualifications
Master’s degree in social work, community counseling, or psychology.
At least 2 years of relevant clinical experience (preferred).
Demonstrated cultural competence and commitment to quality improvement activities.
