NOW OFFERING DAILY PAY - WEEKEND POSITION SUMMARY The primary purpose of the Wound Care Nurse is to provide primary skin care to residents under the medical direction and supervision of the attending physicians, Nursing Supervisor, Director of Nursing, or Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders. You will also assist in modifying the treatment regimen to meet the physical and psychological needs of the resident in accordance with established medical practices and state requirements, and with the policies and goals of this facility. Collaborates with regional staff as needed to work together toward mutual goals.

CUSTOMER SERVICE

Demonstrate positive customer service when performing the role of the Wound Care Nurse with residents, family members, facility staff and medical staff.

Demonstrate sensitivity for residents' rights in all interfaces with residents and family members.

ESSENTIAL DUTIES AND RESPONSIBILITIES Administrative Functions

Confer with the Director of Nursing and/or licensed nursing personnel regarding dermatologic disorders of residents.

Consult with the Care Planning Team concerning assessment evaluations and assist in planning and developing skin care treatment for residents.

Initiate requests for consultation or referral and respond to requests from residents, physicians, or nursing staff.

Examine residents and their records, distinguishing normal from abnormal findings to recognize when to refer to a physician.

Complete medical forms, reports, evaluations, studies, charting, and other administrative duties.

Implement and maintain policies and procedures for skin care treatments; interpret them to physicians, families, and the public as appropriate.

Make written and oral reports and recommendations to the attending physician, Medical Director, or Director of Nursing concerning resident status and care.

Committee Functions

Serve on the Interdisciplinary Care Plan Team.

Work with the team to develop comprehensive assessment and care plans for assigned residents.

Participate in and attend various committees (e.g., Policy Advisory) as required and as appointed by the Executive Director.

Provide written and oral status reports of residents being treated.

Evaluate and implement recommendations from committees related to medical activities and skin care of residents.

Personnel Functions

Cooperate with other personnel to achieve department objectives and maintain good interdepartmental and public relations.

Meet with and solicit advice from the Medical Director, Director of Nursing, and nursing staff regarding dermatologic care of residents.

Maintain a friendly working relationship with health professionals, physicians, consultants, and family members.

Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies.

Medical Care Functions

Identify, manage, and treat skin disorders and lesions, including abrasions, foot problems, decubitus ulcers, infections, scaling and benign tumors.

Perform assessments using observation, inspection, and palpation.

Complete quarterly dermatologic examinations and record results.

Provide therapeutic services and consult the Care Planning Team for care plans.

Deliver treatment for uncomplicated dermatologic disorders under physician supervision.

Ensure residents with decubitus ulcers receive prophylaxis and treatment, including daily inspection, turning, activity, adequate diet, and a clean, dry bed.

Provide direct physical and psychological nursing care in emergencies and during illness or disability.

Assist with admission, discharge, recruitment, and transition of residents.

Assist in rehabilitation activities, monitor physical therapy results, and provide preventative health care, nutrition counseling, and education on skin care.

Staff Development

Participate in in-service training classes on dermatologic nursing skills and procedures.

Conduct educational programs for residents and families regarding skin care at discharge.

Attend annual OSHA and CDC training on hazard communication, TB management, and blood-borne pathogens.

Participate in advance directive in-service training for staff and community.

Engage in continuing education to stay current and maintain licensure.

Safety and Equipment

Assist in monitoring medication, medical supply, and equipment inventory, ensuring adequate skin care products.

Use personal protective equipment such as masks, gowns, gloves, and back braces when appropriate.

Wear or use safety equipment (e.g., back brace, mechanical lifts) when moving or lifting residents.

Resident Rights

Maintain confidentiality of all resident care information.

Ensure residents are treated fairly, with kindness, dignity, and respect.

Provide nursing care in privacy, knocking before entering resident rooms.

Report complaints or grievances to the Director of Nursing and maintain written records.

Investigate all allegations of resident abuse or misappropriation of property.

Honor residents’ refusal of treatment requests, following facility policies on advance directives.

Expense Control – Budget and Planning Functions

Monitor economic conditions and recommend adjustments to the Director of Nursing to maintain quality care.

Recommend equipment needs for the department.

Physical Demands – Physical and Sensory Requirements

Move intermittently throughout the workday; speak and write English clearly; cope with mental and emotional stress.

Use prosthetics as needed for sight and hearing to meet job requirements.

Function independently with flexibility, personal integrity, and effective collaboration with residents, personnel, and support agencies.

Meet general health requirements, including a medical and physical examination.

Engage with ill, disabled, elderly, emotionally upset, and occasionally hostile individuals.

Push, pull, move, or lift specified amounts (blanked as placeholders) and assist in evacuation during emergencies if necessary.

Work Environment

Work in office areas and throughout nursing service areas including drug rooms, nurses' stations, and resident rooms.

Expect frequent interruptions and exposure to residents, personnel, visitors, and government agencies.

Communicate with medical staff, nursing personnel, and other supervisors.

Work beyond normal hours, on weekends, holidays, and on call 24/7.

Handle community and civic health matters, attend continuing education, and manage risks such as falls, burns, odors, and hazardous chemicals.

Maintain liaison with residents and families for planning resident needs.

Minimum Skills/Experience Education

Must possess a Nursing Degree from an accredited college/university or graduation from an approved LPN/RN program.

Must have at least three years of experience as an LPN/RN.

Must hold an active, unencumbered LPN/RN license in this state.
