Case Manager
The Case Manager provides case management services to children, adolescents, and their families. All Agency staff are expected to focus on quality improvement as part of their job responsibilities, in an effort to make the Agency a safe and healing environment for clients and staff. This expectation includes, but is not limited to teamwork, improving service delivery, fulfilling department goals, and other job‑specific tasks that encourage excellence. Essential Duties and Responsibilities
Makes every effort to maintain a "team first" spirit and atmosphere within the workplace, fostering and maintaining appropriate working relationships with the Encompass team and other Agency departments.
Provides case‑management services focused on the client’s ability to succeed in the community, to identify and access needed services, and to demonstrate improvement in school, work, family, and community integration.
Duties may be conducted in person or by phone and will include those designated by Ohio law:
Ongoing assessment of needs
Assistance in achieving personal independence in managing basic needs
Facilitation of daily living skills development
Symptom monitoring
Coordination and assistance in crisis management and stabilization
Advocacy and outreach
Education and training specific to client and family needs
Interventions that eliminate barriers to education and employment
Activities that increase the client’s capacity to positively impact their environment
Completes or assists with treatment plans (ITPs) in collaboration with the client, formulating goals, objectives, and interventions relevant to the client’s diagnosis; and reviews at least quarterly the ongoing medical necessity of those goals and/or diagnoses.
Aids clients in making and carrying out objectives in accordance with their mental‑health treatment plan, as appropriate for the Case Manager role.
Participates in Quality Improvement activities, including peer review and case‑record reviews.
Provides input and feedback, based on observation of clients, to contribute to therapist’s development of mental‑health assessments and ITPs.
Completes timely and well‑written documentation in the CareLogic EHR for all clients.
Attends wrap‑around meetings and family team meetings, as needed.
Attends agency meetings as needed—including staff meetings, clinical or small‑group supervision—to increase and maintain competencies in the mental‑health field.
Engages in ongoing training and maintains licensure and/or mental‑health credentials.
Provides on‑call services, as needed.
Performs other duties as assigned.
Supervisory Responsibilities None.
Qualifications
Education and Experience : High school diploma required; bachelor’s degree preferred, with strong preference for a degree in Social Work or Psychology. Three years of experience in a mental‑health or related field strongly preferred; if no bachelor’s degree, three years of experience is required. Experience working with children and families strongly preferred.
Language Skills : Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, and governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to present information effectively and respond to questions from managers, clients, customers, and the public.
Mathematical Skills : Proficiency with arithmetic, rate and ratio calculations, percent, and bar‑graph interpretation.
Reasoning Ability : Ability to define problems, collect data, establish facts, and draw valid conclusions; interpret technical instructions in mathematical or diagram form; handle abstract and concrete variables.
Computer Skills : Familiarity with Microsoft Office suite, email, and internet navigation.
Certificates and Licenses : Valid Ohio driver’s license; driving record must allow insurability. Ability to maintain licensure and drive a motor vehicle in Ohio.
Physical Demands : Frequent walking or running; climbing or balancing; stooping, kneeling, crouching, crawling; standing or walking for extended periods; lifting up to 50 lbs; tolerance of moderate stress; vision requirements for close and distance tasks.
Work Environment : Occasional exposure to fumes, airborne particles, chemicals, outdoor weather; moderate to high noise levels.

Must be able to commute reliably for all on‑site positions. Some roles require a valid Ohio driver’s license and an insurable driving record.
