Overview The Licensed Charge Nurse (RN or LVN)
will be responsible for ensuring the delivery of efficient and effective nursing care while achieving positive clinical outcomes and resident/family satisfaction in accordance with accepted standards of practice, state and federal regulations, and licensing requirements. The role operates within the scope of practice defined by the state Nurse Practice Act and includes resident care, staff assignments, mentoring, education, and coordination with physicians and other medical professionals.

Responsibilities
Conduct daily nursing functions in accordance with company, state, federal, and local rules, regulations, and guidelines.
Admit, transfer, and discharge residents with appropriate documentation.
Orient new residents and families to the facility.
Participate in scheduled facility surveys (inspections).
Ensure high‑quality clinical outcomes and resident/family satisfaction during the shift.
Serve as a resource for all staff regarding the quality of services provided.
Administer medications and treatments timely and in accordance with facility policy; transcribe and execute physician orders as allowed by the Nurse Practice Act.
Effectively manage nursing personnel on shift to limit overtime and agency use.
Assess nursing staff performance relative to unit standards of care and goals.
Conduct unit huddles to promote communication and provide updates to the Director of Nursing.
Participate in shift‑to‑shift communication between incoming and outgoing nursing staff.
Coordinate shift assignments, resident assignments, rest and meal breaks for Certified Nurse’s Aides.
Collaborate with other departments to provide timely, effective care consistent with individual needs and preferences.
Assume nursing administrative responsibilities for weekend and holiday coverage as requested.
Stay beyond scheduled shift if needed to meet state staffing requirements and/or resident needs.
Ensure timely and accurate compliance with the MDS process and that it reflects the resident’s plan of care.
Ensure Point Click Care is utilized according to company process.
Ensure physician orders are followed as prescribed.
Effectively manage medication supply and handling in accordance with company policy.
Notify the resident’s attending physician and family of any significant changes in health condition.
Report incidents, accidents, and adverse events promptly to the Administrator and Director of Nursing; document, investigate, and address them per company policy and federal/state regulations.
Investigate resident/family complaints and take appropriate actions for resolution.
Demonstrate knowledge and appropriate use of the Company Infection Control Manual.
Evaluate the effectiveness of care interventions, identify problems and trends, and develop alternative solutions.
Other duties may be assigned as required.
Qualifications
Graduate of an approved Licensed Vocational program and licensed in the state of practice.
Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
Current BLS/CPR certification (required for all SNFs; ALFs as required by state).
