The Hebrew Home at Riverdale by RiverSpring Living, a world-renowned long-term care organization located in Riverdale – North Bronx, serves over 18,000 people every day, both in the greater New York Community and on our beautiful 32-acre overlooking the Hudson River.
RiverSpring Living provides a full range of care solutions including nursing home care, managed long term care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.
RiverSpring Certified Home Health Agency provides skilled home services, including nursing, physical therapy, occupational therapy, speech therapy, and social work as well as home health aide services.
The CHHA-Clinical Manager will collaborate with the interdisciplinary team’s Plan of Care to ensure necessary, appropriate and effective care for the patient supervising Nurse Care Managers.
Key Responsibilities:
Develops and implements orientation programs for field staff and ongoing in-service education
Participates in compliance and quality improvement efforts.
Oversees the day to day operations in the absence of the Director of Patient Services.
Provides oversight and supervision to the Nurse Care Managers, as well as other members of the clinical care team as deemed appropriate in provision of care.
Qualifications:
Current New York State Registered Nurse License
BSN degree or degree in Health & Human Services required. Master’s degree preferred
Two 2 years of Home Care experience required
Five (5) years assessing patients for home care safety and developing appropriate care plans and plan adjustments experience required.
Two (2) years supervisory experience required.
Patient Review Instrument (PRI) assessor skills experience preferred.
MS Office required.
Valid driver’s license and proficiency in MS Office required
Compensation:
Annual salary: $120,000 - $130,000
