Responsibilities
Utilize your experience, skills, and education to fully understand and implement the Ryan White Care Act program in your area.
Collaborate with providers and the Healthcare Center team to ensure every Ryan White client receives the support and resources they require.
Provide education and assistance so that clients understand their eligibility and the services available.
Meet clients, conduct Needs Assessments, develop Action/Care Plans, and provide ongoing education about diagnoses and medications.
Re‑engage clients who have dropped out of care and support new patients at high risk of disengagement, using risk‑factor assessments, missed appointment data, and utilization data.
Understand mental health needs and basic needs such as housing, food, and clothing; develop relationships with programs and agencies to facilitate referrals and support.
Conduct ongoing retention calls to ensure continuous care and uphold cross‑division cooperation.
Coordinate care for people living with HIV (PLWH) to achieve and maintain viral load suppression and retain medical care.
Ensure all medical treatments and care plan interventions are coordinated, followed‑up, and completed to provide timely and appropriate services.
Verify clients’ eligibility for the Affordable Care Act program and enroll qualifying individuals.
Advocate for AHF’s core initiatives and embody its mission and values in all client interactions.
Maintain collaborative relationships with multiple leaders within AHF’s organizational structure.
Qualifications
Preferred: Master’s degree in Social Science, Social Work, or Human Services.
Required: Minimum Bachelor’s degree in a social science or health discipline.
Preferred: Minimum of 3 years of Case Management experience.
Preferred knowledge of Ryan White Services.
Knowledge of community resources.
Competence in Cultural and Linguistic Diversity.
Licenses and Certifications: (as required by the appointment).
Schedule Monday to Friday, 8 am–5 pm
