Overview Licensed Practical Nurse (LPN) – Work with the Health Care Team to deliver quality, patient-centered care in alignment with Kaiser Permanente’s nursing mission, vision and values. Provide nursing and administrative support to clinicians and other health care team members.

Responsibilities

Clinical Tasks – Prepare patients for office visits and ensure all test and clinical information is ready for clinician review. Perform focused/limited nursing assessment and triage of routine or self-limiting medical conditions. Use standardized tools to identify and address health maintenance gaps. Provide health teaching and education. Document nursing care using KP HealthConnect and other information systems.

Clinical Procedures – Perform diagnostic tests therapies and procedures within scope of practice. Maintain proficiency in core clinical tasks and procedures, including vital signs, medication administration, wound care, and minor surgical assistance.

Practice Management Activities – Adjust clinician schedules, respond to patient messages via phone, mail or electronic portal, and help coordinate follow-up duties. Assist in coordinating care for patients with complex health needs.

Population-based Care Outreach – Participate in health maintenance screening initiatives.

Teamwork & Leadership – Start workflow, reprioritize as needed, respond to medical emergencies, mentor peers, report hazards, obtain instruction and complete annual competency assessments.

Other Duties – Perform other duties as assigned consistent with job description.

Basic Qualifications

Graduate of an accredited school of nursing.

Practical Nurse License (Washington) or Compact License: Practical Nurse within 6 months of hire. Practical Nurse License (Oregon) within 6 months of hire.

Basic Life Support certification.

Additional Requirements

Demonstrated clinical nursing skills.

Demonstrated interpersonal and customer service skills (assessment required for external candidates).

Demonstrated written and oral communication skills.

Computer literacy and keyboarding.

Problem-solving, setting priorities and good judgment for decisions.

Ability to work independently and efficiently under pressure.

Preferred Qualifications

Two or more years of nursing experience in assigned department clinical area within an ambulatory care setting.

IV therapy experience or recent certification.

Familiarity with documenting in an electronic medical record system.

Current working knowledge and clinical skills in providing routine care, education and advice in assigned department clinical area or completion of appropriate competencies.

Home department: DERM. May support other specialties at NLR, KZS, SKY, and occasionally Portland Clinics.
