Our community is looking for a Scheduling Coordinator to join our team. Responsibilities:

Staffing coordination duties include the accurate and timely communication of scheduling changes between Director of Resident Care (Director of Nursing) and field staff.

Provides shift coverage as needed including providing care and/or medication pass duties as assigned/needed

Is responsible for the coordination of patient visits, maintenance and upkeep of scheduling records and log books.

Maintains a current resident charts with necessary information.

Updates logs and schedule sheets.

Prepares weekly schedule for field staff

Assists in coordinating services provided to patients.

Controls and monitors schedule changes.

Takes on call on rotation and as needed

Accurately process new patient referral information into the computer on a timely basis and create/maintain patient calendar for medical and associated appointments.

Acts as liaison among staff, patients, family, physicians, referral sources and payers

Maintains an ongoing relationship with current, future and former clients, client families, company sales team, employees and outside agencies.

Participates in company quality improvement activities.

Understands and adheres to established policies and procedures of agency, as well as compliance to local state and federal laws.

Answers telephone lines promptly, efficiently and channel them appropriately as needed

Performs other duties as assigned by the Director of Resident Care or Administrator.

Qualifications:

Must have a medication technician certification

High school diploma or equivalent required.

Demonstrates good communication and public relations skills.

Passionate for delivering a world class customer experience.

Internet service with minimum Broad band.

Must be able to travel if indicated for training and/or Discovery Communities/Properties.

Experience with providing resident care and medication administration a must; Medical Assistants is a plus

Experience with staff scheduling
