Part-time Weekend On Call Hospice RN Trinity Health seeks a Part‑time Weekend On Call Hospice RN to work at St. Mary’s Home Care and Hospice, Athens, Georgia.

Weekend on‑call shifts include 12‑hour shifts on Saturday or Sunday, or 8‑hour shifts across Friday, Saturday, and Sunday.

Responsibilities

Provide professional nursing care to hospice clients as prescribed and requested.

Conduct client assessment, care planning, therapeutic interventions, and supervise overall care and outcomes.

Collaborate with the interdisciplinary team, attend weekly meetings, and interact with physicians or hospice medical directors as needed.

Anticipate and manage patient symptoms, assist families with grief/loss issues, and implement hospice care plans.

Support hospice aides and LPN/LVNs when required.

Minimum Qualifications

Graduate of an approved nursing education program.

Registered Nurse licensure in the State of Georgia.

At least one year of experience as an acute‑care nurse.

Home‑care experience preferred.

Experience with EPIC preferred.
