We are seeking a highly reliable and compassionate Private Duty Licensed Practical Nurse (LPN) to provide specialized, one-on-one care for a wonderful middle-aged male patient in Hamden, CT. This is a unique opportunity for an LPN dedicated to consistent, high-quality private care and building a strong patient relationship.
Key Responsibilities The LPN will manage all aspects of the patient’s care, working alongside a dedicated assistant/aide. Specific responsibilities include:
Clinical Care : Tracheostomy Management (routine care, suctioning, and emergency protocols) and G-Tube Management (tube feedings, medication, and site care).
Quadriplegia Care : Assisting with all Activities of Daily Living (ADLs) and monitoring the patient's status.
Team Collaboration : Working closely with the assisting aide to ensure seamless and coordinated care.
Reporting : Documenting care accurately and communicating patient status changes to the family/supervising RN.
Required Schedule (Consistent and Rotating) This role requires excellent attendance and reliability as the schedule includes essential weekly and weekend hours.
Consistent/Weekly Schedule
Friday: 7:00 AM – 3:00 PM
Saturday: 7:00 AM – 3:00 PM
Rotating Schedule
Friday: 3:00 PM – 11:00 PM
Saturday: 3:00 PM – 11:00 PM
Sunday: 3:00 PM – 11:00 PM
Shifts are mandatory as scheduled. Reliability and attendance are non-negotiable for this critical role.
Patient Information
Condition:
Middle-aged male defined as Quadriplegic (has some movement in arms/legs).
Disposition:
Friendly, great personality, and actively involved in his care team.
Support:
An Assistant/Aide is always present to provide support with non-nursing tasks.
Qualifications
License:
Current, active, and unrestricted Licensed Practical Nurse (LPN) license in the state of Connecticut.
Experience:
Prior experience with Trach and G-Tube care is highly preferred.
Screening:
Must be able to pass a comprehensive background check.
