Key Responsibilities
Provide physical therapy services to patients in their homes, ensuring individualized care and treatment plans.
Conduct assessments, develop and implement treatment plans, and monitor patient progress.
Collaborate with healthcare teams to ensure comprehensive care and coordination of services.
Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care delivery.
Maintain accurate and timely documentation of patient interactions, treatment plans, and progress.
Required Qualifications
Must have a valid Washington state license as a Physical Therapist.
At least 1 year of recent skilled home health PT experience within the last 3 years.
Current BCLS certification.
Preferred Experience / Skills
Experience working with adult patients with various medical conditions.
Familiarity with OASIS documentation tools.
Ability to work independently and as part of a team.
