Licenced Vocational Nurse (LVN/LPN)

– San Marcos, Florida

Schedule:

Friday–Sunday, 3 shifts per week (6:00 pm–6:00 am). Flexible full‑time, part‑time or PRN available.

Patient Profile:

Adult client with feeding tube, tracheostomy, and ventilator care (training available).

Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse in accordance with the individual patient care plan and physician orders, while maintaining confidentiality and professional boundaries.

Essential Job Functions

Deliver and coordinate quality patient care in compliance with physician orders.

Continuously observe, assess patient condition and care needs, reporting changes to supervisor and/or physician.

Document all activities, assessments, nursing actions, responses and coordination of care promptly.

Participate in, implement, and update the nursing care plan.

Take appropriate nursing action based on assessment to achieve expected outcomes.

Recognize changes in patient needs and implement interventions to prevent or reduce risk.

Comply with Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.

Use infection control measures that protect staff and patient per OSHA standards.

Educate patient and family regarding disease process, self‑care techniques and prevention strategies.

Maintain knowledge of nursing competencies by participating in continuing education and internal learning opportunities.

Requirements

Graduated from an accredited nursing school.

Current, unrestricted state license as a Licensed Nurse (LPN/LVN) in the state of practice.

Current CPR certification (hands‑on component).

Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care policies.

TB skin test current within last 12 months.

Six months prior hands‑on nursing experience preferred but not required.

Reliable transportation.

Preferences

Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.

Home health experience.

Experience with pediatric care (South Carolina) or specific state requirements.

Ability to travel and lift up to 50 pounds.

Physical stamina for prolonged walking, standing, bending, kneeling, twisting, and moving equipment without assistance.

Visual and hearing acuity; strong sense of smell and touch.

Ability to respond appropriately to emergency situations in the home or during transport.

Comfortable functioning in environments exposing to allergens, blood, bodily fluids, and infectious diseases.
