Department Details:
The Skilled Rehab and Home Health team at Good Samaritan in SE MN seeks to hire a Physical Therapist to join our team. The position is full time, 35‑40 hours per week, M‑F (weekdays/weekends/holidays can be flexible), covering Home Health in the surrounding areas and assisting as needed in our Skilled Nursing Facilities. We treat orthopedic, weakness, balance/falls/gait, cervical/back pain, general deconditioning, and other conditions. We have 2 full‑time PT and 5 full‑time PTAs. New grads welcomed! Paid travel and mileage between home health patients.
Job Summary Plans, organizes and delivers physical therapy programs that help clients, patients, and residents recover and improve mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions for individuals affected by injury or disease. Utilizes tools and techniques to administer interventions following safeguards. Documents client information for evaluation; establishes therapeutic plans and modifies them as necessary. Educates clients and their families about treatment plans and self‑exercises to continue at home. Demonstrates and incorporates current research into daily treatment. May treat individuals of all ages with varying diagnoses. Demonstrates documentation in accordance with departmental guidelines and meets all regulatory requirements. Exhibits flexibility, participates in departmental promotion, and may supervise assistants, aides, and students. Provides ongoing education and training to peers. Performs other related duties as requested.
Qualifications
Appropriate education level required in accordance with state licensure.
Must be licensed in the state(s) of practice as a Physical Therapist.
Valid driver’s license when applicable.
Obtains and maintains required department‑specific competencies and certifications.
Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.
