Job Title: Registered Nurse

Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience
License: Must have an active RN license (no pending licenses accepted)
Certifications: CPR, BLS
Must-Have: Proficiency with HomeCare HomeBase (HCHB) and Oasis within the last 6 months
Responsibilities
Provide, coordinate, and direct the provision of home nursing care according to physicians' orders and agency policies and procedures.
Responsible to the Clinical Manager.
Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Conduct home visits and attend periodic office meetings.
Utilize reliable transportation with a current driver's license and agency-required liability insurance.
Average of 5-8 patients per day with productivity expectations of 30 points per week.
Handle various types of cases, including Start of Care, Wound Care, Follow-up Visits, etc.
Participate in on-call rotations as required, typically one weekday a week and one week a month.
Perform charting with proficiency in HomeCare HomeBase and Oasis.
Must be willing to travel up to a 75-mile radius from the home clinic.
Mileage is reimbursed at the IRS standard rate.
Local candidates accepted at the same rate.
Completion of documentation within 24 hours of the patient appointment is mandatory.
Be open to floating to nearby clinics if patient volumes are insufficient.
