Join the Deaconess Home Health team as a Physical Therapist. Provide in‑home physical therapy services, promoting rehabilitation, recovery, and improved quality of life.

Job Summary Work closely with patients, families, caregivers, and interdisciplinary healthcare professionals to develop and implement personalized treatment plans that address each patient’s unique needs and goals. Focus on improving functional movement in the home setting.

Key Responsibilities

Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.

Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function and mobility.

Educate patients and families about exercises, safety, and lifestyle modifications to promote recovery and prevent re‑injury.

Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.

Maintain accurate and up‑to‑date patient records in compliance with state regulations and agency policies.

Qualifications

Bachelor’s degree from an accredited school approved by the American Physical Therapy Association.

Active registered PT license in the state where you practice.

Current CPR/BLS Certification.

Preferred: One‑year clinical experience, including teaching or clinical setting.

Current driver’s license.

Compliance with federal, state, and local laws and regulations.
