Family Care-Registered Nurse / LTSS Service Coordinator - RN Clinician (RN Case Manager)
Job Overview
This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance. The position is located near Southwestern Wisconsin counties: Pepin, Buffalo, Trempealeau, Jackson, Clark, La Crosse, Monroe, Juneau, Vernon, Sauk, Richland, Crawford, Grant, Iowa, Lafayette, Green. A $5,000 sign‑on bonus is offered.
Responsibilities
Perform telephonic and face‑to‑face functional assessments for identification, evaluation, coordination, and management of member’s needs across physical health, behavioral health, social services, and long‑term services and supports.
Manage members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective and efficient utilization of health benefits.
Obtain thorough member history to develop an individual care plan.
Establish short‑ and long‑term goals collaboratively with member, caregivers, family, natural supports, physicians; identify members eligible for alternative care levels or waiver programs.
Develop and oversee care plans, ensuring member access to services.
Assist with implementation of care plans, facilitating authorizations/referrals as appropriate within benefit structures.
Interface with Medical Directors, Physician Advisors, and Inter‑Disciplinary Teams for person‑centered care plan development; assist in problem‑solving with providers, claims, or service issues.
Minimum Requirements
Associate of Science in Nursing or Bachelor of Science in Nursing with minimum 3 years experience in service coordination, case management, or similar role with chronic illnesses, co‑morbidities, or disabilities; or equivalently qualified education and experience.
Current, active, valid and unrestricted RN license in Wisconsin.
Minimum 1 year experience working with at least one Family Care target population.
Preferred Skills, Capabilities, and Experiences
BA/BS in Health or Nursing preferred.
Strong preference for case‑management experience with older adults or individuals with disabilities.
Long‑term care background preferred.
Commitment to member‑centered care and cultural responsiveness.
Family Care/Partnership experience preferred.
Comfortable using technology.
Strong interpersonal and communication skills.
Ability to work collaboratively in a team environment.
Problem‑solving and crisis management abilities.
Travel to client sites, including member homes and community locations for in‑person visits.
Additional Information
