Delivery of Patient Care
Assumes responsibility for the coordination and delivery of patient/caregiver care. Conducts nursing assessments to assist in the overall management of patient/caregiver needs. When appropriate, develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved. Facilitates patient and caregiver independence to the extent possible. When providing hospice care, appropriately utilizes VIA Health Partners patient related protocols, standing orders and guidelines. Assumes responsibility for conducting effective patient and caregiver education. Encourages and allows time for questions and consultation. Provides education to patients and families as needed. Communicates effectively with members of the Interdisciplinary Group (IDG), patients, caregivers, and other health professionals to ensure continuity of care. Maintains open lines of communication with supervisor. Keeps supervisor apprised of any complex patient/caregiver issues. Applies principles of infection control and follows guidelines for universal precautions in accordance with accepted professional practices.
Interdisciplinary Group
Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences. Establishes and maintains effective working relationships with team members. Identifies specific patient/caregiver needs and makes referrals to appropriate team members.
Documentation
Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and licensure/certification standards.
Professional and Personal Accountability
Demonstrates competency in knowledge and skills. Maintains VIA Health Partners professional reputation and projects it appropriately. Assumes responsibility for professional development and staying current with trends in healthcare; incorporates new information into practice. Maintains current professional licensure, CPR certification and CEU requirements. Maintains productive and professional relations with external agencies and healthcare providers. Exhibits time management skills; plans the workday while remaining able to respond to crises.
Other
May perform other duties as required. (IPU) Cleans patient rooms as required per patient room cleaning procedure and ensures areas are stocked and clean, using proper chemicals and protective equipment.
Qualifications
A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred. A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required. The Nurse Licensure Compact (NLC) allows an RN to practice in other compact states while subject to each state’s practice laws and discipline. Current CPR required; CHPN preferred. Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions; experience in direct care of patients with life-limiting illnesses required (hospice/palliative care; family medicine; internal medicine; geriatrics; oncology; home health care; medical-surgical).
