School-Based Therapist - Fayette County New Vista of the Bluegrass, Inc.•Lexington, KY, United States

The School-Based Clinician will perform a range of clinical services related to the assessment, care, and treatment of children and youth experiencing emotional or behavioral issues, in the school setting on a school-year calendar schedule (10 months per year consistent with the school calendar). The School-Based Clinician is a positive team player that promotes a culture of inclusion and participation.

Required Education and Experience

Master's degree in psychology, social work, marriage and family therapy or counseling or closely related field

Licensed in the Commonwealth of Kentucky

Valid driver's license, reliable operating transportation, and proof of current auto insurance

Preferred Education and Experience

Experience working with children, adolescents and families who experience complex emotional and behavioral difficulties.

Qualified Mental Health Professional

Provide direct clinical services for intake, screening, and referral services

Participate in the client's ongoing assessments, diagnosis and collateral treatment: mental health issues, developmental disabilities, and substance abuse co-occurring disorders

Deliver evidenced based counseling that provides effective medically necessary treatment of mental and behavioral conditions in individuals and families

Conduct comprehensive needs assessment, risk assessment, and full psychosocial history when indicated

Work with each client and family (if appropriate) to develop a targeted, individualized plan of care outlining specific goals and objectives

Monitor the need for continuation of services and/or change in level of intensity of services

Utilize PCOMS (Partners for Change Outcome Management System) in assessments and clinical treatment episodes

Collaborate with the local outpatient office and other service providers as needed to identify an appropriate transition plan, coordinate appropriate follow up services, and assist client in connecting to beneficial community resources upon transition or discharge

Establish Safety and Aftercare/Transition Plan for summer months to include family members and/or significant others when applicable

Deliver services primarily in school setting or where necessary to meet client needs

Document all clinical work and maintain the client's electronic health record according to New Vista policies and procedure

Manage situations involving client risk and provide clinical crisis services to de-escalate crisis situations and promote stability

Manage a caseload that may include diverse populations with a variety of clinical needs

Participate in regular meetings with Clinic Manager and treatment team

Provide referrals regarding psychiatric evaluations and medication management as well as other adjunct service such as Crisis Intervention/Stabilization, hospitalization, or residential services
