Purpose and Scope

Functions as part of the hemodialysis health care team as a Registered Nurse Applicant under the direction or supervision of a Staff Registered Nurse to provide quality patient care in accordance with FMS policies, procedures, and training. Supports FMCNA’s mission, vision, values, customer service philosophy, and Quality Enhancement Program (QEP) including patient satisfaction and continuous improvement activities. Adheres to all FMCNA Compliance Program and FMS patient care and administrative policies. Employees must obtain a nursing license and registration within state regulations or within 90 days of hire, whichever occurs sooner. Principal Duties and Responsibilities

Staff Related

Directs Patient Care Technician’s safe and effective delivery of chronic hemodialysis therapy and ensures compliance with facility policies and federal/state regulations. Delegates tasks to LVN/LPNs, Patient Care Technicians, and Dialysis Assistants. Assesses daily patient care needs and assigns appropriate duties. Monitors staff techniques and adherence to policies. Participates in staff training and orientation of new staff. Attends all scheduled staff meetings. Patient Related – Education

Ensures patient and family education on End Stage Renal Disease (ESRD), renal disease, vascular access, dialysis therapy, and health conditions. Provides ongoing diet, fluid, and medication compliance education. Discusses education related to diet/fluid and medication compliance. Provides detailed education on adequacy measures such as Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), and Urea Kinetic Modeling (UKM). Encourages transplant awareness, modality awareness, and catheter reduction. Patient Related – Dialysis Treatment

Delivers safe and effective care to ESRD patients. Implements treatment prescriptions including Sodium modeling and Ultrafiltration modeling to ensure stable therapy. Assesses patient response and adjusts treatment plan as indicated by the prescribing physician; communicates concerns to Team Leader or physician. Identifies and communicates patient-related issues to Team Leader or physician. Initiates Initial and Annual Nursing Assessment and documents patient needs per FMC Policies and Procedures. Participates in pre evaluation, initiation, monitoring, termination, access homeostasis, and post evaluation per FMC procedures. Intervenes for changes in patient adequacy status and troubleshoots access flow issues per OLC/AMP yellow lights. Supervises and monitors hemodialysis access care per established procedures. Administers prescribed interdialytic transfusions and documents patient response; notifies physician of adverse reactions. Ensures accurate and complete documentation by Patient Care Technician on the Hemodialysis Treatment Sheet. Patient Related – Laboratory-Related

Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System. Prepares lab requisitions for Spectra or alternate lab. Uses correct lab tubes for prescribed specimens and performs appropriate lab draw and processing procedures for all lab samples. Follows through on missed labs and specimens reported as insufficient per company policies. Packages specimens per DOT policies for shipment of blood or body fluid specimens and hazardous materials. Directs and delivers labs to appropriate labs. Reports abnormal lab results to appropriate physician. Ensures lab results are forwarded to physicians as requested. General Duties

Enforces all company approved policies and procedures, and state/federal regulations. Maintains overall shift operation safely and efficiently. Acts as a resource for other staff members. Meets regularly with the Clinical Manager to discuss personnel and patient care status, issues, and information. Collaborates and communicates with physicians and team members to adjust and coordinate care. Provides assistance with prescription refills per FMCNA Policies. Transcribes and enters all physician orders into the Medical Information System promptly. Oversees all patient information documentation. Maintains facility drug list for all required stock medications. Maintains competency with all emergency operational procedures; initiates CPR and emergency measures in case of cardiac/respiratory arrest. Ensures verification and availability of adequate emergency equipment. Provides appropriate vaccinations, immunizations, and annual TB testing. Administers medications per approved algorithm(s) and documents effectiveness. Administers PRN medications and documents assessment of effectiveness. Records controlled substances per law. Assists with patient transportation coordination if needed. Maintenance/Technical

Ensures a clean, safe, and sanitary dialysis treatment environment. Demonstrates competency in operating all dialysis-related equipment safely and effectively. Maintains cleanliness of all patient stations, machines, and chairs. Responds immediately to blood spills per FMCNA Bloodborne Pathogen Control Policies. Medical Records & Documentation

General

Documents all relevant data: physician orders, lab results, vital signs, treatment parameters, and patient status in the Medical Information System. Verifies accuracy of Patient Care Technician documentation. Daily

Reviews and ensures completion of Hemodialysis Treatment Sheets by all patient care staff. Ensures all procedures are followed for opening and closing facilities and that staff and patients leave premises safely. Monthly

Initiates, documents, and completes ongoing Continuous Quality Improvement (CQI) activities including monthly reports. Completes monthly nurses' progress note. Ensures patient medical records are complete with appropriate info and labeling. Reviews transplant status and follows established procedures for action. Creates patient care plans for new or unstable patients within 30 days. Completes long-term program requirements; annually completes initial and annual Nursing History and Assessment physical. Completes Annual Standing Order Review with each physician as required. Other

Continued employment requires obtaining and maintaining all required credentials or certifications in valid status, renewing prior to expiration; notify supervisor immediately of changes. Capable of working in demanding environments, multitasking, and adapting to changing needs. Maintains regular and punctual attendance. Performs other related duties as assigned. Physical Demands and Working Conditions

Provides direct patient care with heavy lifting and ambulation; requires frequent standing and ability to bend over. May lift up to 200 lbs. mechanical equipment with assistance; may lift chemical and water solutions up to 30 lbs. up to 5 feet. Work environment characteristic of a health care facility with controlled air temperature and moderate noise. May be exposed to infectious and contagious diseases/materials. Graduated from an accredited nursing program within the past 12 months. Experience and Required Skills

Required to obtain nursing license and registration within 90 days of hire or state regulations, whichever occurs first. Current appropriate state Temporary Permit. Must meet practice requirements in the state of employment. 0 – 1 year’s related experience. Acknowledgement
