Oak Hill is seeking a highly motivated and dynamic Licensed Practical Nurse (LPN) to assist in maintaining and improving the health and wellbeing of individuals by monitoring health status and providing appropriate treatments to individuals with Developmental and Intellectual Disabilities to enable their highest quality of life in terms of their physical, emotional, and mental health while adhering to Oak Hill's policies and procedures.

About the Position This LPN is required to work Tue‑Sat 3 pm‑10:30 pm (37.5 hours). This position is located in Shelton. The LPN assists in maintaining the health and well‑being of program participants by monitoring health issues and implementing policies, procedures and programs designed to promote health and enhance independence and quality of life, in keeping with Oak Hill's mission.

Minimum Requirements

State of Connecticut Licensed Practical Nurse (LPN) license

Valid driver's license

Experience working in a healthcare or group home setting

Familiarity with caring for individuals with disabilities or complex medical needs

Responsibilities

Provides assistance, guidance and instruction in activities of daily living; including personal hygiene; meal preparation, the serving and feeding of meals; housekeeping activities; social and recreational activities; behavior modification; medication administration and other activities related to maintaining residents'/program participants' well‑being.

Attends to the health needs of residents/program participants by becoming familiar with and maintaining current knowledge of their health issues.

Maintains resident/program participant health by contacting the physician in medical emergencies, writing an incident report, and keeping the Manager or Program Supervisor and Group Home Nurse.

Facilitates resident/program participant care and provides awareness and access to important health related information by taking doctors' orders and recording the information on medication Kardex, medication cards, nursing notes, and in the shift to shift communication book.

Provides information to ensure continuity in care and programming by communicating all medical concerns regarding residents/program participants to the incoming shift, verbally and in writing. (Addresses all medical issues concerning residents under "Nursing Notes" in the medical, to ensure follow-up and awareness.)
