Start Date: End of June / Early July 2024 OB Coverage: Required Regional Anesthesia: Must be comfortable with Scope: No Cardiac, Complex Pediatrics, or Transplants Must be comfortable performing own cases or supervising in a Care Team model Call Requirements: Required Credentialing: 60-90 days Requirements:
Board Certified: Required Licensure: Idaho State License or IMLC Required DEA/CSR: Required COVID-19 Vaccination: Proof required at the time of submission Comfort with Regional Anesthesia: Required Perform Own Cases/Supervising Care Team: Required Other Credentialing Items:
MD diploma Internship/residency/fellowship certificates All medical licenses DEA Life support cards (BLS) Current and previous certificates of insurance Driver s license Professional headshot CME records for the past 2 years Immunization records (TB, Flu, HepB, Tdap, Varicella, COVID) References
